Apply for the Epsilon Chi Chapter Award. You may save your work and return to the form later for up to one week or until the June 1 deadline. Please organize the presentation of the documentation by identifying the selected criteria and writing a brief explanation including pertinent examples which describe how the nominee has demonstrated each of the criteria listed for the specific award. Material to be submitted with the Nomination: (1) Written statement that individual agrees to be nominated for award. (2) Supporting documentation, which addresses specific criteria from peers, faculty, and community leaders. The awards committee will review the quality of the supporting documentation.
