College of Health Sciences Outstanding Preceptor Award

Description

This award is to recognize a preceptor/clinical instructor that demonstrate excellence in collaborative practice while working with students through their clinical or experiential course.

Eligibility

 Must be currently licenses or credentialed to practice and in good standing professionally

 At least two-year work history with COHS

 The service for students is not paid by COHS or ODU

 Preceptors who have received this award in the last three years are not eligible

Application Materials:

1. Completed nomination form
2. Letter of nomination: Nominations accepted from from ODU COHS faculty member; the letter should describe the nature and the time frame of the working relationship with students.
3. One letter of support from a student describing their collaborative practice with the nominee.
4. Resume or Curriculum vitae

Submission and Deadline:

All materials should be converted into ONE PDF file and emailed to the COHS Administrative Center (COHSAdmin@odu.edu) by July 15, 2024.

Please do not submit files separately or send hard copies. It is the nominator’s responsibility to submit the requested materials by the deadline. The committee will only consider application materials requested above.

Awards:

The awardee will receive a certificate of recognition with $500.
