This form must be filed no later than the end of add/drop for the semester proposed. Proposed activity must be approved by Undergraduate Research office before credit can be received. Retroactive credit for activities performed in previous semester may be obtained by using the End-of-Semester reporting forms.
Category 1 (non course-based experience) Note: In some instances, semester research experiences that have an unstructured course CRN associated with them (e.g. independent studies, senior projects, etc.) may be used to fulfill this requirement. Students are strongly encouraged to discuss such proposed experiences with the undergraduate research director prior to submission of these forms.
All applications (with the exception of approved research-intensive courses) must be accompanied by an endorsement from a faculty mentor. Mentors should provide a brief statement indicating they have reviewed the proposed activity and judge it to have a significant component of novel discovery and/or development of advanced research/scholarship skills. Mentors should also indicate their role in mentoring the student as well as their approximate time commitment to the activity. Mentor endorsement forms can be found here.
