SAP Appeal - Maximum Timeframe Academic Plan Form
Students must maintain satisfactory academic progress to remain eligible for financial aid. As part of SAP evaluation, we review the cumulative number of credits a student has attempted. Students will fail SAP when their total credits approach 150% of the published length of their educational program. Academic Advisors: The requested information below should include the remaining courses required for the student’s degree completion. Your signature ensures its accuracy. Please return this form to the financial aid office as quickly as possible. Note – Students must contact a financial aid counselor prior to withdrawals to determine effect on aid eligibility and a new plan may be required. Withdrawals may result in loss of aid eligibility.
Students should only take classes to complete degree. If additional semesters are needed, please attach another page.
I have discussed the terms and academic plan with the student, so that the student can meet the Satisfactory Academic Progress (SAP) standards to maintain his/her financial aid eligibility towards the degree the student is pursuing.
