Advising Referral Form (*Main Campus Only)

The purpose of this form is to change a student's intended major and refer to the new major of choice. This is NOT a major declaration form.

If a student wants to change to ODUGlobal, a campus change request form (not this referral form) is necessary.

Steps to completing the Advising Referral Form:
1. Student connects with current advisor (or success coach/advising hotline/SSC) to discuss major change. 2. Advisor discusses key information pertaining to new possible major choice (GPA requirement, math, foreign language requirement, etc.) to assist student in decision to change intended major. 3. If student wants to continue with major change, advisor will complete the advising referral form and submit.

Advising Management Note:

*Once the major changes, student's prior advisor will be removed and student should now appear on the Unassigned Students list for the new department in Advising Management."

**If you do not see the student on the unassigned list, they may still be attached to the former advisor and can be found on the "View All Advisees Who Have Selected a Different Major" list.
